Pre-Procedure
Port placement was delayed because the patient needed platelet transfusion first (thrombocytopenia, platelets ~45K at diagnosis). Platelets were administered prior to the procedure.
Clinical Context
Dr. Bijay noted that central line is not required for azacitidine + venetoclax chemotherapy — the port was placed for convenience/long-term access. Treatment could have started via peripheral IV.
Dr. Bijay also wanted to speak with the patient one more time before starting treatment to ensure informed consent: "I wanted to talk to her one more time before we started treatment just to make sure she understood everything. I couldn't see her before I left — she was still in radiology suite."
Post-Procedure
Treatment (Cycle 1 of Aza-Ven) was planned to start the same evening or the following morning (Dec 10). Dr. Bijay: "Medically there is no difference between starting tonight and tomorrow morning." Treatment initiation was coordinated via voice call (10 min) on Dec 9.
