Interpretation

Both kappa (36.81, ref 3.3–19.4) and lambda (26.73, ref 5.71–26.3) free light chains are elevated, but the kappa/lambda ratio is normal (1.377, ref 0.26–1.65). This pattern indicates a polyclonal elevation of free light chains, which is consistent with:
- Inflammatory states
- Renal impairment (though creatinine was normal)
- Hematologic malignancy with reactive immune activation

Importantly, the normal ratio rules out a monoclonal plasma cell process (multiple myeloma, primary amyloidosis, light chain deposition disease). Combined with the negative M-spike on Spep, monoclonal gammopathy is effectively excluded.

This result was initially reported as PENDING on the preliminary lab report but was finalized on the same accession's final report.
